Community Based Alternative Services

Community-Based Alternative Services (CBAS) are offered to eligible older adults or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization.

CBAS begins with a comprehensive assessment of the individual’s needs. Care managers will provide or coordinate professional nursing services;Β physical, occupational and speech therapies; mental health services; therapeutic activities; social services; personal care; nutritional counseling; and transportation to and from the participant’s residence and the CBAS center.

Eligibility Requirements

  • Member of San Francisco Health Plan
  • 18 years of age or older
  • Assessment of members physician’s history and physical information and prescription medications
  • Client must be willing to conduct a face-to-face meeting and a needs assessment
