Enhanced Care Management services are offered through CalAIM and Institute on Aging (IOA) for adults transitioning from nursing facilities, adults living in the community who wish to remain home, adults living with mental health or substance use, and adults experiencing homelessness. We also provide community
support services for those seeking housing, and those who need housing adapted to meet their physical needs.

Care Managers can:
- Communicate with a care team including: doctors, mental health providers, specialists, pharmacists, case managers, social services providers
- Find doctors and get appointments for health-related services
- Better understand and keep track of medications
- Set up rides to and from health care visits
- Find and apply for community-based services based on needs. This includes housing supports or meals to fit health needs.
- Secure follow-up care after hospital discharge
