Community Programs

We operate a variety of programs to meet the unique needs of the communities we serve.

Activity Resource Centers (ADRC)

We oversee multiple Activity Resource Centers which enable people of all ages, incomes, and disabilities to access a wide array of Long-Term Services and Support options in the community.

Community Based Alternative Services (CBAS)

We completes initial and reassessments of referrals made by SFHP who are interested in seeking out services at one of the San Francisco County CBAS sites.

Clinical Collaborative

We provide Clinical Supervisors to support Care Management teams throughout San Francisco to help improve the knowledge, skills, and performance of case managers working with seniors and adults with disabilities.

Elder Abuse Prevention

We provide education, outreach, advocacy, and develop strategic partnerships to prevent and respond to the abuse of older adults and adults with disabilities.

Support in Independent Living

We contract with permanent supportive housing partners to provide onsite support to their staff and residents through robust programming.

Support at Home

We provide home care subsidies for middle income San Franciscans to help cover the costs to have access to caregivers.

Temporary Respite Caregiver Support

We utilize short term grants to provide respite care, support groups, and information & referrals for individuals providing caregiving services.
