Social Services Agency (SSA) is a Federal program that administers support and aid consisting of retirement, disability and survivor benefits to aging adults.

Program Staffing

Care Managers, Supervisors and Manager

Program Services

Provides Intensive Case Management to individuals at risk of institutionalization or in acute settings
Purchase services to help maintain safety and independence
Connect clients to all available community resources, stable housing, and appropriate healthcare services to support their ability to live at home safely

Eligibility Requirements

Resident of Santa Clara County
18 and older
Individual willing to live in the community with appropriate supports
Have demonstrated a need for service and/or resource that will serve to prevent institutionalization or enable community living
Assistance needed with at least 2 ADL’s or 3 IADL’s
Medical conditions must be able to be managed in the community
300% of federal poverty level (fpl): <$37,470 annually, Savings/assets of up to $6,000 & Current Medi-Cal status
